Since each plan for each insurance company is different, please check with your company before making an appointment to make sure that massage therapy is covered under your plan. Some plans require a co-pay to be paid to your practitioner, and most require a prescription or referral from an M.D, N.D, D.O, or D.C. Premera requires the prescription to have a diagnosis code, number of visits, and duration of service. Please be sure that your physician knows this and writes the prescription correctly. Please bring your prescription and insurance card to your first appointment.
